Your First Visit
Your first visit to the office is for your initial checkup. During this visit, a doctor will review the forms you have filled out to ensure we have as much information as possible to make an accurate assessment of your case. The checkup also consists of an examination and evaluation of your spine, nerve system, and posture. When combined with the other elements of your checkup, it enables us to provide you with individualized care plans for precise and effective care.
A health intake form is completed at or before your first visit to Patient Forms our office. (These forms are available here on our web-site or we can fax, mail or e-mail them to you prior to your first visit.) It is imperative that these forms are filled out completely and accurately because they inform the doctors of your current complaint and your history of injuries and illness.
You and one of our doctors will review your health intake forms together. A history of spinal impacts, including auto-accidents, work, birth trauma, recreational and sports injuries will be reviewed, along with your current work and daily activity. We will also uncover lifestyle habits affecting your spine’s alignment including any chemical, emotional and physical stressors. You and one of our doctors will also investigate your most pressing health concerns.
Clinical Examination & Evaluation
Your wellness evaluation will include a nervous system examination and a thorough postural evaluation. These tests will help determine if subluxations are affecting your health and body function.
